Partner visa extension and switching to 5 years route at the same time

Hi,

I have been granted Partner route on 2021 but was on 10 years route because I didn't meet English requirement

I have met the requirements now and my current visa is coming to an end soon, I would like to Extend my visa and also switch to 5 years route.

Is FLR(FP) or FLR(M) the correct form to apply?

I am using form FLR (M) to fill my info and there is a question

is this the first time you apply as partner or extend
I chose NO
then Reason for extension
I don't know whether to choose
First extension as a partner (following an initial period of entry as a partner, or as a fiancé(e) or proposed civil partner of a settled person)
or
OTHER and then say, this is my first extension but I also want to switch to 5 years route from the current 10 years

I have a question as well regarding financial, I am self employed, but it's not a year yet so I don't have the tax return for the year, and this is the first year I am doing self employed, how do I proof it? is it just bank statement?

also they asked for 2 years bills as well, is this needed if I already have partner visa in 10 years route?
