Section 6.3 in Appendix 1 asks for details of this TEST where

Awarding Body is ok...
Title is ok....
Level is also ok....

But m confused about
Date of Award??? There are 2 dates on IELTS test report form. First 1 is the date of exam(24 july 2008). and second is date (1 aug 2008) BUT result was declared on 6th of Aug 2008 ( which is not mentioned anywhere in the test report form). So what out of above 2 shud be date of award?

Award Reference number??????
Appendix 1 , allows me to fill in a number with max 12 digits but IELTS test report form number has 18 digits. how to get the solution for the same problem???

i have the same questions as you. Try contacting your exam centre for the details. I have sent an email to the examination centre asking them to clarify my doubts. In my test report form there are just two dates, date of exam and another date, which appears to be the date of award. How did you get to know the date the result was declared? I just received the result in post when I was in London..I would say if it is not written anywhere then you better not mention it,unless someone confirms in writing that it is the date of award.

Also try www.ielts.org/contact_us.aspx, they have one normal form (which gives you a page not found error once you enter all your contact details and your query :) and another 'Enquiry form', which I will try only if my test centre does not respond in few days.I also asked another question regarding the awarding body. It is not clear whether it is the 'British Council' or the 'Univ. of Cambridge ESOL examinations'(I think its the latter but not sure)..
