Skilled worker visa application

Urgently an advice needed.

Can someone help me with few queries as i have started my application and my visa expires in a day.

I am filling out skilled worker visa application:

1: I will be on new entrants (£31,000 for 4 years) but I can’t see this option in application form?

2: Medical Treatment? Does this include illness like depression or any other small nails on going treatment?

3: Parents D.O.B ( as it was 1/1/year now it got change to 1/7/year) I previously have used 1/1/year when i was applying for student visa?

4: Driving offence ( Recently received 3 points and £100 fine due to entering into no entry road) do i need to mention it or not?
