Name Change of Institution

Hi Friends,
I completed my Bachelor Degree in Bharath Institute of Science and Technology, Chennai in 2004 under University Of Madras. Now my college name has been changed to Bharath Institute of Higher Education and Research which has become a deemed university in 2006.
In Self Assessment tool i could find my college name as Bharath Institute Of Higher Education and Research (New Name of my college).
In my Medium of Instruction Certification my college name has been mentioned as Bharath Institute of Science and Technology.
Please guide me what i have to do ? I need to get any certification to prove both are same ?

(1) If the name of the Institute (Changed name) is there on the points calculator why are you submitting the medium of instruction letter?

(2) What you rather need to do is - get a letter from the Institute that it was known as XXX before and you did complete your degree from the Institute
