Got a query about Tier-1 (Gen) Dependent application.

This is Question B4 in Section 2 of dependents appl. form:
B4. If the dependant is known, or has ever been known, by any other name(s) than those given above, please give details below and send the evidence showing the name change:

Does this apply to a woman who's last name(surname or family name) is changed from maiden to a married (middle and last) name, after marriage?

Yes that's true, I think I saw this in Muslim families, where they change girl sur name after she got married, so you need to show the marriage cetificate when she had her real name, then second proof willbe her passport which will have her changed name...

I was anyway going to provide marriage certificate to provde the relationship as a aspouse in dependant's application form.
