Tier-1 application-application form-need help

Dear all,
I have started preparing my documents for Tier-1 visa. Application to be submitted in IV week of Feb 2009. My points are 95 (50 for education & 45 - earnings) I have some concerns in writing application form which are listed as follows:

1) POINTS BASED SYSTEM FORM (VAF9 NOV 2008)
PERSONAL DETAILS -The point 6.3 says - Have you ever been refused a visa for any country including the UK?
I have got one US visa rejected on 27-Oct-2005 due to the reasons i donot know ( I feel its because of improper funding documents).Now the question is ,do I need to write this in my application form since its an old information and it appears in my Old passport and not in the current passport)

2)The point 6.4 says Have you been granted a visa for the UK in the last 10 years?
I have been granted two visas one Switzerland visa on 01-12-2006 to 28-02-2007 and the second Schengen Visa on 11-03-2007 valid for 10 days. The answer to the point 6.4 is clearly a "NO" . However if I write Granted visas details in point 7.1 as an additional detail to support my application?
Will this 'additional information' help positively?( as compared to Visa rejection point 6.3 as expained above)?

3) APPENDIX 1: TIER 1 (GENERAL) MIGRANT
SELF- ASSESSMENT point 3.2 Please indicate your method of degree verification Put a cross (x) in the relevant box
Qualification on the UK NARIC database :
How do I know if my Qulaification is appearing in the NARIC database ? Can we check this database on our own?

4)Is it a must to show previous earnings details of 12months or 15months? I can safely show salary slips of 11months but 12th and 13th months salary slips is a problem.(Since i was on my wedding leave for 1 month and also i switched my job.)

I will be highly obliged if you could answer my questions and guide me in the application forms.

Qualification on the UK NARIC database :
How do I know if my Qulaification is appearing in the NARIC database ? Can we check this database on our own?

Use the point based calculator and in education qualification section follow the instructions and give the detials and if you find your university, degree and subject than it means ur degree is prsent in NARIC DB

Note that you can use any 12 consecutive months in the last 15months... from any no. of employers with breakups.... read the guidance for more info... but if you get the 45points even using 11months than you dont need to worry and can go ahead with those 11payslips..

Your reply came in just few hours , I was amazed to see that experienced people like you, help each other so quickly and professionally...inspite of knowing each other...... Thanks once again.
Slowly I am building confidence for Tier -1 visa application ,by solving all my qerries ...I think i will get clear idea of all the doubts which may arise in due course. Since I am applying on my own , I think this forum will help me a lot...
I am still waiting for the answers of first 2 points of my post. I am little worried about them....I am waiting for answers from the knowlegable people like you...
