Help with Dependent Visa Extension when the main applicant has got the ILR

Hi everyone - I am trying to fill Visa extension form for my partner who has completed 4.5 years in the UK on dependent visa. I have got ILR after completing 5 years on skilled worker visa (I had 3 years of visa then it was extended to another 3 years). I have a few questions on the application form so I really appreciate your help:
1. The form has not asked for the duration of the visa so not sure if I am doing something wrong or because it will by default considering the remaining time required for ILR.
2. What entry clearance or permission to stay in the UK do you have? - This should be Skilled Worker Dependent Partner Leave To Remain ?
3. Does the main applicant already have their Skilled worker visa? This should be Yes or No because I had skilled worker visa before but not now
4. What is main applicant UAN? It should be ILR UAN or visa extension UAN?
5. In the travel history section, it asked for more than 2 weeks absence which I have filled but my partner was also away for one week where this should be filled?
6. Did you receive medical treatment in the UK? here Do I have to fill for regular GP appointments as well which are related to regular sickness?
7. Which evidence documents should be uploaded? Bank statements, bills or anything else as well?

Which form exactly are you completing??

A Skilled Worker dependent visa extension is for 3 years.
Char (CR001 not Casa)
