Do I fall into the HSMP Forum judicial review category?

Do I fall into the HSMP Forum judicial review category?

This is my situation:

HSMP
   Date of Issue: 14 APR 2005
   Valid Until  : 12 APR 2006

HSMP Extension
   Date of Issue: 28 FEB 2006
   Valid Until  : 12 APR 2009
As I applied for HSMP in 2005, and then my extension in 2006, I feel that I fall into the current category:

"Migrants who currently hold HSMP leave and have either applied for an extension of stay or will need to do so in the future"

Hence I would need to fill in this form:

Am I correct? Or shold I fill in the Tier 1(General) further leave form?

You do fall under HSMP JR, and you need to fill FLR Tier 1(HSMP) form,
