need urgent form help

hi, i am filing for my wife's visa (i am a hsmp visa holder). Can someone please let me know the correct form for this visa? everytime i call VAC they give me a different form number! my hsmp visa was granted in december 2007 and i think the vaf10 is the correct form. Pls help!!!
