New PBS (T-1 general Form query

I have a friend who raised a question on the new PBS Tier 1 form - when used for re-instatement under the HSMP JR, after answering question G1 and G2 of section 3A (p25) where shall he go next?

The guidenotes are not much help as it tries to fit in a lot of applications in a single form he says...

any help will be great.
