Which form to use to apply for hsmp extension?

I applied for hsmp in may 2007 and got visa valid from Aug 2007. Now going to apply for extension and not sure which form to use for extesnion. Please guide whether should I use "flrtier1.doc" or "tier1generalapplicationform[1]".

For HSMP extn to t1 you can download the form from the link below:
