HSMP(old rules) dependent visa

Hi

I am presently in UK under HSMP VISA (not tier 1).I wanted to apply dependent visa fro my wife who is in India now. can you please guide me for the following

1. Which application form i need to fill
2. Visa application fee?
3. How much maintanance funds i need to show in my account
