Need help for self employed documents for HSMP extension

Hi all,

i am having my entension for HSMP in August 2009, which is now Tier 1.
I am working in a company as salaried person as well as self employed in my limited company, as director of the limited company. I am drawing dividend from self employment, what document do i need to submit for self employment ? is dividend voucher sufficient or need to produce accounts as well ?

Guys please help me out asap.

read guidance notes. every thing is clearly mentioned there.you need to have dividend vouchers
company accounts (profit & loss signed by director)
accountant letter confirming details of those accounts

Tell HO that you are salaried person and dividend is a part of your renumeration package.You need to have the following:

1. Monthly Personal Payslips.
2. Personal Bank Statements (showing Salary+Dividends)
3. Dividend Vouchers.
4. Letter from Accountant (mentioning Dividends +Salary -- Monthly Breaksup)

Hi Thanks Fahad,
I Have shown dividend in my accounts but have not taken actual dividend in my bank account, my salary income is ok that agrres with my payslips.
let me know what should i do ?

what u mean 'I Have shown dividend in my accounts but have not taken actual dividend in my bank account'

you need dividend vouchers which shows your gross and net dividend. and net amount must corroborate in bank statement.
