hsmp > tier 1 extension -- what form ?

HI

Came on hsmp in 2007. Due for tier 1 extension. Main applicant+3 dependants.

Firstly, which form do i use ?

Secondly, Do i need to fill a seperate form for each dependant ?

Finally, do wage slips and bank statements have to be sgined and stamped ?

1. U need to include all the dependents in u form and also fill seperate form for each dependent which are avaialble on the UKBA web site.

2. Original bank statements( u receive in the post) does not need to be stamped

3. Read the Page 25 of the guidence as it explains about the requirements for the wage slips.

So even though I add my dependantas on the tier 1 form, i still fill individual forms for them as well ?!?

OK, if so, what form do i use for them, is it tier 1 dependants ?
