No. As long as amount matches in payslips and bank statement it is fine.
2(c) on few occasions my gross salary is same but net salary is different, for e.g aug gross is 850 and net is 800 and July gross salary is also 850 but net salary is 790? is that a problemNo. As long as amount matches in payslips and bank statement it is fine.
2(d) on couple of occasions i've an extra entry "others" on my payslips and amount credited is for e.g 100.00, this other entry is an additional entry with Gross salary, so i 've two entry one is gross and one is "others"? is this a problem b/z i think my accountant made some mistakes in previous months by paying me less and he rectified it by paying extra next month and showed in the payslips with "others" entry No. As long as amount matches in payslips and bank statement it is fine.You can also take letter from your accountant for the same.
2(e) i've 3 different types of tax codes on my last 12 month's payslip? is this a problem No. As long as you are paying tax you are fine. Well..to be honest your payslips amount should match bank statement
2(f) if your employers pays you via OTR(online transaction) is this an issue? No. Hope description of the transaction links the transaction back to your employer. If not explain it in your cover letter.
3> in my last 12 month transactions the first few months i was overdrawn but last 7-8 months are ok and well above 2-3k, i am using the same account for maintenance and earning proof? any problem? No. as long as you have sufficient maintenance funds in your account.
4> also 7-8 months ago i had a single transaction of 9k, will they ask me to explain this large single transaction? No. They don't bother about the money in your account.
5> in 2003 my first application for entry clearance was refused then i made a fresh application and got entry clearance , i did mention in my new application that i got refusal last time, then i came to UK and had 3-4 times visa extensions but here in UK i never mentioned that visa refusal and never had any problem, currently i m on Tier1 PSW, do you think on my tier 1 general application i should mention that refusal which happened 5 years ago? Yes. You should provide all details required by form. Do not hide any details which can haunt you in future.

Refusal: I remember somewhere in form, it does ask you whether you have been denied visa in past. I might be wrong but Can you please check again. If I were you and if it is not asked anywhere in form, then i would not provide any additional info which could go against me.
