Question Regarding Multiple Visa/ employer for ILR

All,

I came on work permit in nov 2004 and worked in that company untill August 2006. In August 2006, I resigned and another company applied for my work permit. They received the approval letter and I started working for that company.

However, I never sent my passport for stamping their work permit and then in October 2006, I joined another company who had appied a fresh work permit for me and I have got that work permit stamp on my passport.

Then in June 2008, I applied for Tier 1 General Visa and got that. So I complete 5 years in nov 2009 and applying for ILR now at croyden. Couple of Question

1) Is the non stamping of my second employer VISA on passport an issue. Though the approval letter stated that I have 6 months to get it done as my existing leave to remain was valid.

2) Do I need to mention it to case worker or in SET O form any where?

3) I was eligible for ILR in nov 2009, will I get my ILR backdated or not?

Answer is No, one of my friend eligible in June 2009 but the ILR stamped on Nov 2009,
