application rejected, ask some question about appeals

My friend apply for Tier 4 student, but rejected, want to ask some question about appeals

Here was the situation:
The visa expired at October/09, he graduated in Jule/09 from Bsc, then from aug/09
started ACCA and planned to start MSc at Jan/10

So when he sends the application form, he included the visa letter of ACCA and a unconditional offer letter for MSc course.

Because he forgot to include Id card attach to the application form, when HO request the ID card, he sent his Id card
and his current visa letter with it(he has finish ACCA by then and the visa letter was not
the same Uni as the previous unconditional offer)

His application was refused:
It says on the letter that his course fee has been paid (proofed) in full (ACCA's course fee amount)
but the proof of 1600 pounds for maintenance has not given

His ground was that:
1) He did include his bank statement which has money more than 1600 pounds
(the bank statement was an internet banking print out with bank letter head, name(Surname and initial) and stamps)
Because he put two bank statement attach to the application, one is his, one is his brother's stating that he will sponsor
if necessarily. Both bank statements has more than 1600 pounds.

In the refusal letter it mention that a private sponsor is not acceptable, so is it possible that HO thought he only supply
bank statement of his brother but not his, hence the rejection

2) when calculating the maintenance amount, it says inner london 800*2, but he actually studied outside of london and that is what he putted on the application form

Questions:
1)except the appeal form, are there any document should be supplied? e.g. passport? new bank statement?
2)on the appeal form, should he include what he is currently studying, and supply visa letter of current studies. It seems HO only take into account of the ACCA course he did.
3)if the appeal is refused, is it possible to appeal again?
4)Is post hearing better or oral hearing
