Plz how to get this document on section 9(b) new form

Hello guys
My current state of visa:
First arrival in UK on 1 June 2005 as a work permit holder (10/05/2005-10/05/2010)
Visa will be expired on 10 May 2010.

I am so worry about my current situation in which I am trying to apply for IRL as a work permit holder. In fact, I do not understand some part on SET (o) form and I would like to get advice from you guys follow as:

QA) on section 9, (9B) there are two documents required
1. Employer’s letter to prove that applicant is still need.
2. What is this document? company to prove that applicant has spent a continuous period of 5 years in the UK. So do I need to tick this box or not?

QB) Am I eligible to apply for ILR visa because it will be 4 yrs 11 months 4 day if I send my application on 5th May 2010, which means NOT COMPLETING 5 YEARS.
Please, if you have experience on applying visa before, you might understand how most people are worry about this matter especially before sending their application form. So please help me for my questions with clear evidence answer. my plan is that I will go to Croydon 5th May, and then i will let you know my result.

Yes, you need to apply 28 days in advance. You entered UK on 1/6/05 and complete 5 yrs on 31/05/10, you can apply any time on or after 3rd May.

1. You need current employer's letter, one format is on this forum as a sticky, plesae search
2.Yes, you need to tick this and provide previous emplyer letters or P60s

Others with 5 yr WP in similar situation may kindly confirm.
