I will have to check, I have posted the letter format in of the threads in the same forum. Dont remember which thread that was.
Will search and post.

Just write an application to Bank branch manager saying that you hold an account with them from XXXX till date need this for showing maintenance funds for 3 months period XXXXX to XXXXX for Tier -1 visa and that your min amout balance in that period is XXXXX which is above required 2800 pounds.

Also make sure that they dont write one day account balance, as its mentioned by UKBA that it is not enough.
