New English requirement ???!!!

Hello

My friend is going to apply for a spouse visa soon and is confused about the English test requirement. She has done TOEFL and on the UKBA website, it says that TOEFL is accepted but i have some queries to that :

1) what if the TOEFL was done before 3 years ?? is that gonna be ok ???

2) Will they accept a printout of the result ?? or they need a certified copy ?

3) Is Life in the UK test accepted for this english requirement ??
