Question on Institute name in the application

Hi,
I have question on institute in the application. I have studied in a private college that is affiliated to Andhra University, The original degree shows only university name. What should I write in the institute place in application.
Do I need to give any details like address and phone numbers in the covering letter?
what should I fill in the grade? We do not have any grading system.

what kind of notes we can write in the additional information?

Please reply as I have an interview on this Friday 08OCT10.

Degrees usually mention the name of the institute as well (down below in the text), I am not sure why is that the case with yours. Do you want to have a re-look?

Contact details at the institute & University? Yes, by all means.

Grade is usually the letter Grade like A+, A- etc. or Honours, I div / II div etc.

Notes in the additional information section: Only if there is any info that you think is relevant for the case and you believe has not been captured in the application form (including due to lack of space) should you provide information in the said section.

So Should I write both institute and university name in the specified column and mention these detials in additional information?

Just write your University name, no need to mention the insitute name. I have had a similar case, and i just wrote the name of university. Hope it helps
