Sample Appointment Letter & Contract

Hi

I have been working since Jan 2006 but my employer has not given me anything in writing, he is saying he would sign the documents and I can compile them myself.

If anyone has a sample of Appointment Letter & Contract I would change is accordingly.

Further I would like to know that can a permanent position be of six months as my salary is 15k per annum but my current is just initailly for 6 months.
