I am planning to apply for HSMP.
Currently I am on WP for 3 years in UK.
1 year WP (previous employer) + 2 years (current employer)

1) Do I need to send Original Work Permits while submitting the application?

2) In Part 2 of HSMP Application Form:
What do I need to fill in Q7 - Q13 and which evidences I have to provide?
