Tier 1 extension - Independent Contractor

Hi All,

I am an independent contractor doing IT Consulting. I have set up my own company. I take month 1500 as my salary and 2000 as dividends.
I can produce documents like P & L accounts, dividend vouchers from my accountant.

When applying for extension what documents I need to produce as I am director of the company. how much income (salary + dividend) I need to show?
