My Salary and the Minimum Fund ( 2800 Pounds) are in the same account.

Should I get two back statments ?
1. Statment for 12 months period to match with pay slip
2. Statment for 4 months to match the fund maintenance.

Your statement for the last 12 months would, by definition, also include the last 4 months, right?

No need to provide 2 copies of the last 4 months if thats what your question was.
