Spouse ILR - Application Dates; Bank Statements

My wife is a US citizen currently living in the UK on a 2 year leave to remain granted in September 2009, I am a UK citizen, and as we're going to have to apply for ILR for her this year we've started researching into what we have to do. I would appreciate help with a couple of questions which have come up.

1. Application Dates The UKLBA website states you can apply from 28 days before the end of the initial 2 year period, but the visa adds 2 months on the end of that period before it expires. So as my wife's visa was granted on 1st Sept 2009 does that mean we can start applying in July 2011 (based on the initial 2 years) or we have to wait until October 2011 (taking into account the extra two months). We think the UKBA website is very confusing on this, and we're just second guessing ourselves and each other.

2. Bank Statements My wife has bank accounts in the US still, so do we have to provide statements for those? If so, the bank only has online statementing, so we're not quite sure how we'd go about getting original paper statements. Has anyone had problems with this before.
