I want to applies Tier 1 (General) Depended visa for My Baby

I want to applies Tier 1 (General) Depended visa for my baby. I am living in United Kingdom under Tier 1 (General). My Wife is already on my depend.
we are living last 2 year and 2 month in London. I have Tier 1 (General) extantion on November 2011. My Baby born on 15/01/2011.

1. For My Baby depended Application which visa form I will Use?
2. How Much of the Visa Fees?
3. Which document I need to send to the Home office?
4. In application form can I use baby Thumb as a Applicant signature. or I will sign for My baby.
5. how much bank balance I need to show on my bank statement?

You can use Tier 1 dependant Visa form for your baby

Post - £500, PEO - £800
3. Which document I need to send to the Home office?
3 months bank statement, all 3 passports, marriage certificate, birth certificate of ur child, completed application form, 2 photos, 3 payslips [optional], PBS certificates
4. In application form can I use baby Thumb as a Applicant signature. or I will sign for My baby.
you have to sign for ur baby
5. how much bank balance I need to show on my bank statement?
