Paperwork for dependent visa

I am planning to apply depending visa for my wife and i am on tier 1 general been living here since 4 years. my questions are....

A) How many months coucil tax bills show it as a prof for accomidation?

B)Does double bed room shared accomidation will be okay to show?

C)How much funds i have to maintain in the bank & how many months show it minimum?

D)Which application form, would i have to use to apply for my dependent?
