For getting Tier 1 General Extension for Self & 2 Dependants at PEO Cardiff:-

Can you please confirm each of the following

1) While fixing Appointment on https://ukbabooking.homeoffice.gov.uk/ukba/index.start
the Form type to choose is "PEO Tier1 / Biometric Online" Right?

2) Exact Form to Fill for self?

2. Yes
3. IMHO dependant i.e. spouse and children under 18 years would be included in the main applicant form (if applying at the same time). Only children over 18 years or when applying seperately would use the PBS dependant form.

Don't confuse between PBS forms and settlement forms!

Each dependant must complete a separate PBS (Dependant) form
