a letter from accountant self-employed

Hi,Guys
I am self-employed from 2008.Applying 4 ILR, HSMP pre nov 2006(now Tier1(G).
Could anybody please share the template of accountant's letter or at least advise what to mention in it as my accountant never wrote such letters.
I will really appreciate any help
