Tire 1 General Extention What is the previous workere ref No

Hello:

I am going to apply my visa extension very soon, during fill in the the application form, I have a question which is I am not sure.

B12: Give details of any current or previous worker reference number under work permit arrangements(if applicabl)

I had a work permit visa nearly 4 years ago. Do I need to fill in the worker reference number? If I do, where can I found it, is it in the work permit visa? Thank you very much

Call home office and get your reference number from them directly.
Normally your reference number start with alphabetic and then follow with numbers. Your surnames first letter is your first letter of this reference number.
