Need Urgent help on Dependant visa of tier1 general

i want to apply tier 1 dependent visa currently i am currently on Psw visa.what proves we need to submit as we are living together.

we got..... pay slips.surgery registration,bank statements, driving license on same address. tenant agreement.nikha nama.

do we need to provide photograph?.... and other communication proof ....as they are not required at home office web site.

we both working full time and both have maintained bank statements.

What is the date of divorce (as in decree absolute)?

it is 12 april 2010 on divorce paper
