Tier 1 general renewal

Hi all,

I'll soon be renewing my Tier 1 General Visa, I work as the sole director of a limited company receiving salary and dividends. My queries are as follows:

1. Does anyone have a link to what a 'formal' payslip looks like cos my accountant sends me monthly slips which i think look formal but cant be too sure?
2. If at all the payslips need to be stamped and signed, who does that?
3. My payslips state salary paid by cheque however I normally do a BACS transfer, do i need to change this?
4. Do I apply as 'employed' or 'self-employed'?
5. How do i show evidence of tax payment?
6. Apart from payslips, dividend vouchers, personal bank statements and letter from my accountant, do i need to submit any other documents to show proof of earnings in this case.

2. They don't need to be signed but it depends on the format.

Like I wrote, I only work through my limited company as the sole director (not formally registered as self-employed) and receive monthly salary and dividends from the same company. What group does this fall under?

Also, I have looked at the guidance notes and if applying as a company director, i think its just those 4 documents i need (salary slips, dividend vouchers, personal bank statements and letter from my accountant), I'm just not sure if other company documents like financial statements need to be included.

You'll be employed by the company. Self employment relates to sole traders and partnerships.

The link above is my type of payslip (with no letter head), is this a formal payslip which needs no signature and stamp?
