EC for spouse of HSMP visa holder-List of docs

Hi,
While applying for entry clearance for the spouse of an HSMP visa holder, is it required that one must produce a CERTIFIED copy of the passport of the HSMP visa holder ?. Is it okay if the passport copy is not certified. ? Is it okay if the passport copy does not contain the entry stamp at the port of entry at UK ? HSMP visa holder is working in UK.


Also please let me know if the following list of documents is enough.
Is it a must that I include document 8 below ?.

1. Certified copy of the passport of the HSMP visa holder
2. HSMP approval letter copy
4. Marriage certificate
5. Childs birth certificate
6. Offer of employment of the HSMP visa holder.
7. UK Bank statement.
8. Copy of Utility bills for the house at UK.
9. Passport of the spouse and child
