Contractor (own company)+ JR + 5yrs. Need Help with docs

Hi Guys
I am looking for a feedback from any contractor working through their company in relation to docs for applying for ILR.
I am a contractor and to begin with unsure about the Set O form that whether i need to download it and fill and submit or fill it online?
I would like to apply in person. What is the appointment booking time line for it?
