Please can any one let me know how to write in the review .....i mean how to start......

can any one send me the sample of the same.... i have got the form from the website.... but i want to know that how to explain them and how to start the explaination........

You can start like this:

Explanation letter

Dear HSMP team,

My name is xxx xxx and I applied for HSMP in xxx, 2006. My reference number is xxx. Recently I received a refusal letter with the explanation of the causes for my refusal.
