ILR with HSMP

I am due to apply for ILR ON 23th nov 2011. My hsmp started on dec 2006 but had the letter of approval before 7 th nov 2006. So i am covered by the hsmp judicial review -5 years.
My questions-
1. Do me or my wife need to take the lifein the UK test.
2. I am appying in person, does my wife need to attend the appointment.
