Help on FLR (M)

Hi all,

Just a few quick more questions on the form FLR M, hoping someone can help!

Question 1.4 - Your full name as in your passport or travel document
The name in my passport has my surname first but my letters/bank statements have my surname last - which one should i use?

Question 8.2 - Rental
Our rental is £600 but we split 50% with another tenant. How much should i write? Our tenancy agreement states £600 though.

Income each month:
Does this include estimate of bonus and overtime or just my annual basic?

Also, on page 24, Photographs and documents checklist - do i have to complete this section?

I have also got a speeding SP30 FPN - should i declare this?
