What to Include in ILR set(M) application?

Hello! I am due to apply for setM Indefinate Leave to Remain as a spouse of a UK citizen and I need help to make sure I have all the paperwork I need for UKBA.

I have:
a) Passed the Life in the UK test.
b) nearly completed my 2 year stay (ends 14th December 2011)
c) Six statements (utility bills, bank and savings statements) showing our address with both my husband and my name on it.
d) two passport-sized photos of myself and one of my husband.

What am I missing, if anything? Please, I can't find what else to send - do I need to write a statement - do I need to make copies of documents? Do they need to be signed by a notary?

I would be most appreciative if someone would tell me what else I should include in my application.
