Tier-1 Gen Extension

Hi,

I had initially come in WorkPermit and then changed to Tier-1 Gen.
I am now going for an extension. There are questions regarding my initial travel in the application form. Do I need to fill in the workpermit details there?
Also where can I find the Home Office immigrant reference number?
