how to obtain extension to spouse when she is abroad

Hi All
Congradulation to all who got the extension for another 4 years while I am waiing for the same.
Meantime can anyone tell me how to obtain extension when spouse is still abroad. Whether she has to travel first on visist visa to UK and then apply? I have not claim points for her education .
What happen to children older than 18 years old? how to get extension for them while they are studying out of UK? However they were below 18 yrs when appply for HSMP and got the visa for 12 months.
Can you please explain these to me?
