Out of UK letter from Employer

Can anyone send or share the UK absence letter.

I am planning to prepare letter which includes

S.No
Date of exit
Date of entry
Number of days
Total No of days
Reason


Salary UK/India

Example as below :

S.No : 1
Date of exit : 21st Dec 2007
Date of entry : 6th Jan 2008
Number of days : 15 ( travel date excl)
Reason : Holiday
Salary UK/India : UK


Will this be OK ?

I can ask my company to provide this data on the company letter head ...will it work ?

you dont need a company letter for your personal absences ... if you wish to do so, well and good as your absences are well within safe limits ...

I wouldn't want to show any trips with salary in India ... unnecessary complication.
