tier 1 dependent form - change name

On tier 1 general dependant form they asking for change of name evidence.

My wife's sure name changed after marriage . She never had any passport before marriage to reflect her old surname and her passport made after merriage reflect her husband name as surename..

What sort of evidence do I need to supply would marriage certificate would be enough (keep in mind marriage certificate reflect her old surname before marriage but doesn't display her new surname)

Would it be of any help to provide her national identity card along with wedding certicate as identity card reflects her new surname while marriage certificate reflect her old surname where the citizenship number mentioned on both is same?

it doesn't matter. A married woman can change her name after the marriage to her husband's surname - the marriage certificate with your surname is therefore proof of the reason for the name change.
