Hi there,
I've just submitted my T4 extension application in Dec 2011 and have done biometric. My wife's on T4 which is about to expire. She's in 21 weeks of pregnancy and other medical conditions such as fibroids and placenta not in right place. Because of those conditions, it's not possible for her to take more than 12 hours flight.
I will explain conditions in writing with evidences to UKBA. Is there any other things I should submit to UKBA to switch her visa in UK, e.g. which form to be used for application, etc.?

what course are you studying, how long is it and are you studying at a university or private college?

Does your wife have evidence from her doctor that she cannot travel? What is her nationality?

At the moment, I'm doing MSc at Kingston Uni which is from Jan2012 to Jan2013.
Yes she has letter from GP and other results from hospital, and is from xxxxxxxxx.

when in January does the course start and finish? is it at least 12 months long?

The rules for tier 4 dependents do not permit switching from tier 4 to tier 4 dependent from within the UK. The evidence regarding her medical condition needs to be very strong in order to show that it would not be possible for her to fly. You say she cannot fly for more than 12 hours - the UKBA may say that she could take an indirect flight and stop somewhere for a few hours - you need to have something to rebut that.

Has your wife finished her course of study?

I would strong advise you to see legal advice on this application before proceeding.
