2 Questions in the form, please help

I got 2 questions here.

(1) In the form it asks you to write down any name you have ever been know of. My nickname is Tim, people call me Tim because my original name to hard to pronounce, but this name Tim is not my official name, should I put that down in this column?

(2) My professional referees are both Optometrists working in the same company, can I use them both for my referees? Will that count as related?
Of of them is Irish, one is British, so should I leave the box blank in the Irish referee passport number section?

1) i believe it is best to put down the nickname Tim especially if there are any documents or letters to you with the name Tim in there. Or if many people call you Tim. It doesn't hurt to put it down.

2) as long as they are not blood or marriage related, that is fine....even if they had the same surnames but no relation to each other that is equally fine.....my referees were both professors in the same university and that was no problem. as long as one fills in his passport number, the irish referees passport number is not necessary.

1- Mention Tim as other names you're known by- its important.

2. Referees seems fine to me but don't take my word on that.

1)"Tim" is not ur official name then why u want to put on the form? There is no need of it...

so if i do put my nickname down in the box saying 'any other names you have been known of', should I put it something like this

TIM (NICKNAME)

it that ok?? as the guideline say you need to explain why you have such name??

If people call you in the office by Tim which is different from your birth name then you should mention it in other names. However, if Tim is short for Timothy for example, then there is no need. From my experience, don't hide anything, its better to give them all the information you're asked than hiding it and get in trouble later on.
