Extension of Tier 1 - Latest Form

All,

Just so that we all are in the same page and not getting confused post 6th April changes ...

can anyone please provide the link to the form that needs to be used while extending your Tier 1 visa.

Now that the deadline for changes has passed, there is only one form indicated in the Tier 1 (G) section of the website and that is the current form for all applications as of now.
