EEA family permit 5 years after

Hi!

I live in UK for over 5 years (I hold a registration certificate)
My husband nearly 5 years his Residence cards will expire in 2 months.

During that period I was employed ( just one company from 2004 until now) but twice I was on maternity live and until november 2011 I was working just 10h weekly. From november I have second job.(I work over 40h)

1.questions
If my husband want to apply for permament residence (EEA4) do I have to work full time or just be employed for that 5 years? What document are the most important?

2.
Do I have to apply for permament residence as well?

3. My surname and address is different than on registration certificate what I should do with that?

4. Do I have to add my children to aplication?

5. do my husband have any other option? To stay in UK?

6. when should we send aplication?

1. No need to work full time. Just provide 5 P60s or a letter from employer confirming your employment.
2. You don't need to although as you need to send your passport + your 5 years evidence, you effectively just need to add an application form to apply.

3. Not an issue as log as you the same person

4. You can although if they EEA national themselves, I would not bother. Doesn't give them any value.

5. He can apply for another 5 years Residence Card but better to apply for PR if you can.

6. As soon as your partner completes 5 years of residence in the UK irrespective of the expiry date of his Residece Card.
