Tier 1 extension as Limited company: Do I Need payslips etc?

Hi All

I have following query regarding Tier 1 extension due in next couple of months.

1. As a director of my company, who will provide Payslips, is it accountant?
2.Should I check ' Employed OR Self-Employed' while filling the application form ?
2. Along with payslips and dividend vouchers, Did you need to submit the company accounts too (Bank statments and Invoices and PL account) which they considered?

2. Along with payslips and dividend vouchers, Did you need to submit the company accounts too (Bank statments and Invoices and PL account) which they considered? Business account bank statements are must; Invoices, payment remittance are additional documents, but not necessary.
