Quiery re Documents proving cohabitation of Spouse on set(o)

Hi there,
I have a premium appointment for ILR on Thursday,
Has few Quieries..
1) in documents required it says if you have filled section 2 of the Set (O) form re dependents .. applicant must provide proof showing cohabitation with Spouce for a maximum of 2 yrs.
1)I have lived in Uk since 2005 and got HSMP in 2007 dec..My wife has been living with me since 2009 May and then i applied for tier 1 extension in 2009 December .. at that time she also got Tier 1 extension till dec 2012. Her Passport during 1st stamp in 2009 May and during 2nd extension in 2009 dec was in her maiden name but then in 2010 at the time of renewal she has to change last name to My last name. both the passport are attached together we travelled many times since then but no problem ever. Her extension visa sticker is in Old passport on her Maiden surname. does this make any problem at the time of Settlement application?
2) to prove cohabitation I am providing documents we received in joint names like 3 yrs council tax letters, 4 half yrly water bills from 2011 jan to july 2012 and bank statement for joint current/savings account since 2010 march till oct 2012.. would this be sufficient ??
Sorry to make it long and complicated ..Please reply and help as my application is on thursday thanks.

I personally do not think there would be any issues on either questions. However, take EVERYTHING you can get hold of that could be useful for the application.

Marriage certificate (if you have) would be handy, though not sure it will be asked for.

Also could you please confirm..
1) for tier 1 to Settlement route do we need to provide maintenance funds.. I could not find it anywhere saying its required in SET (O) form.. I have however kept last 4 months savings account bank statements in case they ask for..
2) Just to confirm ..date entered in country is the date i got my HSMP granted 1st time in Dec 2007 ?? as it was in country(UK) application i got approval in country and switched my status from student to HSMP category. I went on holiday then in 2008 feb for 1 month and came back in March 2008.
3) DO we need photocopies of all the originals? it only says to bring originals with the application form.

No harm in bringing it along.
