what are the evidences required for urgent treatment?

Hi,

Please share the evidences required to apply for urgent treatment of HSMP application?
Should we also send any evidences, when we fax the urgent treatment forms?....

Urgent treatment is for in country applicants only.

To correct myself. Out of Country applicant can use the urgent treatment form where there is an offer of employment.
